Proctored Examination Request

This request is addressed to a person with an occupation for which a professional oath or code of ethics is an integral requirement. (Examples include medical doctor, dentist, lawyer, librarian, minister, police chief, school principal, etc). A person to whom you are not related by family, marriage or similar presumption, asks that you facilitate the taking of an examination for academic purpose. The examination is to be proctored, that is, it will take place within your observation or that of trusted staff, and will be returned directly to our Registrar in a prepaid-postage pre-addressed envelope.

The person taking the examination (in this particular subject matter) is not to have notes, reference books, pocket calculator, or other aids present during the examination - unless specifically directed (e.g. "calculator permitted") by written instruction. Upon conclusion of examination, you agree to sign and return a form - a sample of which is below.

Form letter for affirmation in postgraduate degree award context

Note: please use official academic or research institution letterhead and sign with your title of Professor or Associate Professor (adjunct where relevant). Assistant Professors are occasionally designated as collateral- or even as primary panel members, when professionally acknowledged for special expertise in a topic.

Please simply address the letter per below, without addressing it to a specific individual. The below affirmation text block is an example, and the actual text used needs to reflect the exact situation. If in doubt, communicate with the ad hoc chair of the approval panel or the Registrar for assistance in the matter.
